Fuel Tank Safety Phase 1 & 2 Training Course

EASA Decisions 2009/006/R and 2009/007/R issued in March 2009 amended existing Acceptable Means of Compliance material of Part M and Part 145 to account for Fuel System Safety Issues. This included organisations acting under these parts being required to undertake extensive staff and management training programmes. This course is designed to satisfy the Phase 2 training requirements of both EASA decisions.

Ageing Aircraft Structures

In 1977 a B707-321C aircraft suffered a catastrophic failure of the right horizontal stabiliser on approach to Lusaka. The resulting investigation uncovered a fatigue induced failure of the horizontal stabilizer structure which should have been fail safe. This led to the introduction of the damage tolerant structure requirements for airplane structures.

In 1988 a B737-200 aircraft operated by Aloha Airlines lost 18’ of fuselage upper lobe skin structure in flight. The resulting investigation uncovered a fatigue induced failure and corrosion were the primary causal factors. This led to the introduction of a large number of Ageing Airplane Structures Rules.

In 1990 an Air India B747-200 suffered the loss of the number 1 engine strut assembly – the first of a series of 5 similar accidents which took place over a 2 year period. The resulting investigation uncovered a lack of structural integrity in the strut design which later required significant modifications.

Each of these accidents was attributed to structural failures which have subsequently resulted in a significant rule changes.

The aim of this course is to summarise the causes in an effort to learn lessons for the accidents and to review the rules which have arisen from the investigations as well as review the resulting structures programme changes arising from the accidents.

Extended Range Twin Operations (ETOPS) Special Operating Rules

Special Operating Rules are not specifically defined in Airworthiness Requirements: the term Special Operating Rules is a collective one intended – for inconvenience – to embrace the following set of Rules:

  • Extended range Twin Operations – ETOPS
  • Reduced Vertical Separation Minima – RSVM
  • All Weather Operations – AWOPS
  • Minimum Navigation Performance Systems – MNPS

This course has been designed to familiarise delegates with the concepts underlying the rules as well as the rules themselves and to assist operators in the setting of policy and procedures for the effective airworthiness management of applicable aircraft.
